Place your Team DVD order with your Credit Card/Paypal.
How do I order the DVD for the Team? The Coach, Team Manager or other designate from your team can order as many DVDs as you need. (Individual Families order here.) Please send PRIMA the following for every player: Parent Name Parent email address (used as Log In) Player Name (Get the Team Order Form here.)	Watch the videos on your smartphone at practice.
How much are the DVDs? $15.00 each (Access to SkillsZone is included for no extra cost.) The DVDs are then mailed to the Coach or designate and there's a $9.95 total S&H charge per Team Order. Whether you order 2 or 18 it's $9.95 total for Shipping & Handling.
How do I make payment? Payment may be made by Check(s) or On-line (above) with Credit Card/Paypal. Contact Angie Eastridge in the SLSG Office for reimbursement from your Team Account. How do I get a the passwords for SkillsZone? Each player's password will be emailed to them directly once the Parent Email Addresses are received.
